  • 2019-20 Incoming 9th graders or transfers! Welcome to Foothill Tech! If you are interested in playing sports at FTHS, please complete the interest form at the link below so we can contact you about the May 1 sports meeting, tryouts, and programs. The team webpages will be updated over the next couple of months with summer info.

Attention all students interested in playing sports in 2019-20:

  • ATHLETE CLEARANCE: Prior to trying out for or practicing with a team, students must submit Athletic Clearance documentation, which includes a sports physical clearance dated within the past 12 months. Begin the online clearance process at https://registermyathlete.com/login/ and use the instructions found here. If you are unable to complete clearance online, please contact the school office for a packet at 289-0023 x1016.

  • SPORTS PHYSICALS: As part of the athletic clearance process, students must submit a Preparticipation Physical Evaluation Clearance, signed by a medical provider and dated within the past year. If your physical will “expire” during the season of sport, you will need a new physical before that season begins. Sports physicals are valid for one year, therefore we highly recommend scheduling sports physicals annually in May/June, so you will be cleared for the summer and entire schoolyear. See below for sports physical opportunities that support the Foothill Athletics Program:

  • SPORTS PHYSICAL OPPORTUNITIES:

    –  Wednesday, May 1, 2019 @ FTHS, 5pm-7:30pm, bring $25 cash or check payable to “Foothill Athletics Boosters” or pay online.  **Bring the completed physical evaluation form. Parents must accompany students.**  Physicals will be performed by doctors & residents from CMHS.

    –  UPDATED: Saturday, June 8, 2019 @ Ventura Orthopedics, 3525 Loma Vista Rd, Suite C (upstairs), 8am – 11am, $25 cash/check payable to “Ventura Orthopedics.”  Proceeds will support FTHS Athletics. **Bring the sports physical clearance form and completed physical evaluation form. Parents must accompany students.**

  • IMPORTANT SUMMER ATHLETICS MEETING, Wednesday, May 1, 2019 @ FTHS:

       5:00 to 7:30pm  SPORTS PHYSICALS will be offered on campus, $25 cash/credit/check payable to “Foothill Athletics Boosters” (first come, first served)
    5:00 to 6:30pm   In-n-Out Burgers for purchase ($7/meal for a Hamburger/Cheeseburger, Chips & Drink), join Boosters, buy spirit wear, complete athletic clearance! (You can pre-order your meal online at https://squareup.com/store/foothill-technology-boosters/)
    5:45 to 6:20pm   Parent meeting for incoming 9th graders and/or new to sports  – Spirito Hall
    6:30 to 7:10pm   Session 1 individual team meetings (fall & winter sports)
    7:20 to 8:00pm   Session 2 individual team meetings (winter & spring sports)

  • SUMMER TRYOUTS: Check www.foothilltechnology.org (Athletics menu), for summer tryout dates & schedules; sports can hold tryouts for incoming students beginning June 17.  Students who choose not to participate in summer practices/tryouts will need to tryout when the season of sport begins, during the school-year; course schedules will be rearranged at that time if they make the team.

  • SUMMER PRACTICES:  Some teams will hold summer practices/tryouts, though all sports are required to have a 3-week “dead period” during the summer. This is a good time to make vacation plans. Summer practices are not mandatory but are highly recommended and help coaches coordinate your initial course schedule.  Fall Sports have priority during summer if times conflict.  Summer schedules will be posted soon on the team webpages.

 


  • Athletics Donation: We are asking for each family to please donate $50.00 to the Booster Fund to help alleviate the stress from each individual coach to fundraise for equipment, CIF meals, tournaments/invitationals, recognition/awards, and coaching education/certification. Your tax-deductible donation may reference our non-profit 501(c)(3) tax ID#47-1828368. Cash or checks payable to “Foothill Athletics Boosters” can be brought to the FTHS front office, or donations may be made online. Thank you for your support!

Game Admission Prices

Admission prices for Girls/Boys Volleyball and Girls/Boys Basketball games (both home and away)*:

a)    General Admission (age 12 & over) – $6

b)    Senior Citizens (age 55 & over) – $3

c)    Students of competing schools with valid ID – $3

d)    Children (age 6-11) – $3

e)    Children under 6 years old – FREE

Purchase an ATHLETICS EVENTS PASS at the Student Store that can be used for admission to all FTHS HOME games:

STUDENT pass, $20: Admission to all volleyball and basketball home games, except during CIF play-offs/finals (approx. 30  games, a $90 value!)

ADULT pass, $40: Admission to all volleyball and basketball home games, except during CIF play-offs/finals (approx. 30 games, a $180 value!)

*There is no admission charged for other sports, except during CIF finals.


Letters and Letter Jackets: Letter Jackets may be ordered online at zoomid.com. Click “Design Jacket,” then search for our school “Foothill Tech” to access our school design. Samples are available to try on for sizing at our local spiritwear vendor Proforma Color Press, 1860 Eastman Ave., Unit #104, Ventura, 9:00 AM – 4:30 PM (805) 642-7504

If you have earned a varsity letter and are purchasing a jacket, we recommend ordering the letter with your jacket.

If you earned a varsity letter and would like to purchase just the letter patch, these are available at the Student Store for $5.25, just bring your varsity letter certificate to purchase.

Concussion Return to Play Protocol

After a concussion diagnosis, this graduated return to play protocol MUST be completed before you can return to full competition. CA State Law AB 2127 (effective 1/1/15) states that return to play (i.e., competition) cannot be sooner than 7 days after evaluation by a physician (MD/DO) who has made the diagnosis of concussion. Please bring this form to your physician after a suspected concussion.

Personal Vehicle Use Form and Requirements

Please complete, attach a photocopy of your driver’s license, registration, and proof of auto insurance, and turn in to coach or athletics office once a year (or upon renewal of license, registration, or insurance). This permits you to transport students when the team travels. New paperwork must be submitted each year.

Student Transportation Permission Form

This form must be completed if a parent wants to drive his/her child, or have his/her child drive to or from practices, games, events, etc. when District Transportation is not provided.  Coach and Athletic Director must sign.

Sports Physical CLEARANCE Form

Prior to trying out for or practicing with a team, students must submit Athletic Clearance documentation to the office or their coach, which includes a sports physical clearance dated within the past 12 months.

Sports Physical Evaluation Form

May be requested if you are being evaluated by someone other than your regular medical provider. It should not be submitted to the school.
