ATHLETE CLEARANCE: Prior to trying out for or practicing with a team, students must submit Athletic Clearance documentation, which includes a sports physical clearance dated within the past 12 months. NEW for the 2017-18 school year, online clearance is now available! Begin the online clearance process at https://home.registermyathlete.com/org/CA/00509/ and use the instructions found here. If you are unable to complete clearance online, please contact the school office for a packet at 289-0023 x1016.

Saturday, June 10, 2017 @ CMHS Centers for Family Health, 138 W Main St E, 9am-12pm, bring $25 cash or check payable to “FTHS Athletics.” **Bring the sports physical clearance form and completed physical evaluation form. Parents must accompany students.**
Monday, June 19, 2017 @ Foothill Tech HS, 4:30-6:30pm, $25 cash/check payable to “FTHS Athletics.” **Bring the sports physical clearance form and completed physical evaluation form. Parents must accompany students.** Physicals will be performed by doctors & residents from CMHS.
IMPORTANT 6/19 ATHLETICS MEETING: Incoming parents & students should attend the Summer Athletics Meeting at FTHS on Monday, June 19, 5-8pm for general info and team meetings. A new parent orientation will be from 5:45-6:30pm, followed by individual team meetings at 6:30-7:15pm and 7:15-8pm. Dinner by In-N-Out will be available for purchase 5-6:30pm, pre-order your meal today! Sports physicals will also be offered on-campus 4:30-6:30pm for $25, all proceeds supporting FTHS Athletics.

TRYOUTS: Check the team webpages for summer tryout dates & schedules; many sports will hold tryouts for incoming students beginning June 20. Students who choose not to participate in summer practices/tryouts will need to tryout when the season of sport begins; schedules will be rearranged if they make the team at that time.
SUMMER PRACTICES: Most teams will hold summer practices/tryouts, though all sports are required to have a 3-week “dead” period during the summer. The dead period is a good time to make vacation plans. Summer practices are not mandatory but are highly recommended and help us coordinate your initial course schedule. Fall Sports have priority during summer if practice times conflict.

Sports Physicals: As part of the athletic clearance process, students must submit a Preparticipation Physical Evaluation Clearance, dated within the past year. If your physical will “expire” during the season of sport, you will need a new physical before that season begins. Sports physicals are valid for one year, therefore we highly recommend scheduling sports physicals annually either in June or August, so you will be cleared for the entire schoolyear and the summer either prior or after.

Game Admission Prices
Admission prices for Girls/Boys Volleyball and Girls/Boys Basketball games (both home and away)*:
a) General Admission (age 12 & over) – $6
b) Senior Citizens (age 55 & over) – $3
c) Students of competing schools with valid ID – $3
d) Children (age 6-11) – $3
e) Children under 6 years old – FREE
Purchase an ATHLETICS EVENTS PASS at the Student Store that can be used for admission to all FTHS HOME games:
STUDENT pass, $20: Admission to all volleyball and basketball home games, except during CIF play-offs/finals (approx. 40 games, a $120 value!)
ADULT pass, $40: Admission to all volleyball and basketball home games, except during CIF play-offs/finals (approx. 40 games, a $240 value!)
*There is no admission charged for other sports, except during CIF finals.
Letters and Letter Jackets: Letter Jackets may be ordered online at zoomid.com. Click “Design Jacket,” then search for our school “Foothill Tech” to access our school design. If you have earned a varsity letter and are purchasing a jacket, we recommend ordering the letter with your jacket.
If you earned a varsity letter and would like to purchase just the letter patch, these are available at the Student Store for $5.25, just bring your varsity letter certificate to purchase.

Concussion Return to Play Protocol After a concussion diagnosis, this graduated return to play protocol MUST be completed before you can return to full competition. CA State Law AB 2127 (effective 1/1/15) states that return to play (i.e., competition) cannot be sooner than 7 days after evaluation by a physician (MD/DO) who has made the diagnosis of concussion. Please bring this form to your physician after a suspected concussion.

Parent Pledge Cooperation among coaches, athletes, parents, and school personnel is essential if students are to realize the values of athletic participation. Like coaches and athletes, parents must make commitments to the athletic program to assure such cooperation. This pledge is a component of the athlete clearance process.
Personal Vehicle Use Form and Requirements Please complete, attach a photocopy of your driver’s license, registration, and proof of auto insurance, and turn in to coach or athletics office once a year (or upon renewal of license, registration, or insurance). This permits you to transport students when the team travels.

Student Transportation Permission Form This form must be completed if a parent wants to drive his/her child, or have his/her child drive to or from practices, games, events, etc. when District Transportation is not provided. Coach and Athletic Director must sign.
